Default When is it time to ask for a raise in small office?

I have been working as a part-time secretary/office manager for a surgeon for over a year now. I started off with a good salary which was higher than average. I enjoy my job, feel I am working well but have not been offered a raise yet. Do you think it's too soon to ask for one? I don't want to bring in any bad vibes since we seem to get along well. I also find it a little uncomfortable seeing I am her only employee and she treats me well (still pays me for days when she is away, etc.) Should I just wait a bit longer or not expect a raise in this situation?
