| I feel like I'm being jerked around by the HR people at my place of work. I was given a package that was 300 pages long and told I didn't need to read through it, just fill in the form and return it. It slipped my mind for awhile and I missed a deadline that nobody told me existed. I've now been penalized and must pay more money and submit a "statement of health" to receive any life insurance. I don't know what a "statement of health" is and nobody seems willing to help me with this at all. I apparently am required to apply for coverage as part of my contract with my job, but I don't feel the need to have life insurance as I have no dependents and I'm covered in my family's insurance. What is a statement of health? Can I waive this coverage? |

| The statement of health is a form the insurance company gives you, and you have your doctor fill it out. You can ask them if you can waive it. If they pay 100% of the premium, then the IRS requires them to insure you. THEY get in trouble, if you don't comply. |
